Understanding Back Pain

Back pain is one of the most common reasons Australians seek physiotherapy. It affects roughly 1 in 6 adults at any given time and can range from a brief acute strain to long-term chronic pain that affects work, sleep, and quality of life. The term “back pain” covers a wide spectrum of presentations — from muscular strains and disc injuries to nerve referral pain like sciatica.

At Hopevana, we treat the full range of back pain presentations, including lower back pain, sciatica and nerve referral pain, disc bulges and herniations, postural back pain in desk workers, and lifting injuries common in WorkCover patients. Whatever the cause, our clinical approach starts with finding the root of the problem — not just managing the symptom.

How Hopevana Treats Back Pain Differently

Most physiotherapy treatment for back pain focuses on where it hurts. We focus on why it hurts. Using NeuroKinetic Therapy (NKT) movement screening combined with hands-on manual therapy, our approach finds the underlying movement dysfunction driving your pain — and treats that, so the pain doesn’t keep coming back.

1

Comprehensive NKT Movement Assessment

Your first appointment includes a detailed NKT movement screening. NKT (NeuroKinetic Therapy) is a manual muscle testing system that identifies which muscles in your body are firing correctly and which ones are under-firing or over-compensating. For back pain, this often reveals that the back itself isn’t the actual problem — it’s working overtime to compensate for weak glutes, restricted hips, or poor core sequencing. NKT finds the compensation pattern; treatment then targets it directly.

2

Hands-On Manual Therapy

Once we’ve identified the dysfunction, treatment is hands-on. This includes manual joint mobilisation, soft tissue release, fascia work using IASTM tools where indicated, and targeted release of the over-tight muscles compensating for inhibited ones. You feel the difference immediately — most patients report meaningful relief from session one.

3

Targeted Exercise to Restore Function

The final piece is a focused, progressive exercise program designed to retrain the inhibited muscles to fire correctly. This isn’t generic “do these stretches” advice. The exercises are specific to your individual movement findings from the NKT assessment. Done consistently, this is what makes the result stick.

Patients often tell us they’ve seen multiple physios for the same back pain over many years without lasting improvement. The NKT approach is different because it asks “why is this happening?” — not just “where does it hurt?” For most uncomplicated back pain, patients see meaningful improvement within 4 to 8 sessions, though acute injuries can respond faster and chronic patterns may take longer.

Types of Back Pain We Treat

Six distinct presentations — one consistent assessment-led approach.

01

Lower Back Pain

Acute strains, chronic stiffness, postural pain

Lower back pain is the most common presentation we see at Hopevana. Whether it’s an acute strain from lifting, ongoing stiffness from desk work, or chronic pain that’s never quite resolved, our NKT-based approach finds the movement pattern driving it.

02

Sciatica & Nerve Referral

Pain radiating down the leg, numbness, tingling

Sciatica is back pain that refers down the leg via the sciatic nerve. Our assessment determines whether the source is disc-related, muscular (piriformis), or referred from the spine, then targets the actual cause with manual therapy and progressive nerve mobilisation. 03

Disc Bulges & Herniations

Confirmed disc injuries with imaging

Disc bulges and herniations require careful, progressive treatment. We coordinate with your GP and any imaging findings, focus on safe load progression, and avoid aggravating positions while restoring movement and strength around the spine.

04

Postural & Desk-Related Back Pain

Office workers, prolonged sitting, “tech back”

Hours of sitting create predictable patterns of inhibited glutes, tight hip flexors, and overworked back muscles. NKT screening finds the exact pattern in your body; we then treat it and give you targeted exercises to break the cycle.

05

WorkCover Lifting Injuries

Workplace strains, twisting injuries, repetitive load

Back injuries from lifting, twisting, or repetitive workplace tasks are common WorkCover presentations. As a SIRA-approved provider (Approval No. 15827), we treat workplace back injuries with no out-of-pocket cost and complete all AHTR submissions on your behalf.

06

Chronic & Complex Back Pain

Pain present for 3+ months, multiple previous episodes

Chronic back pain often involves multiple overlapping factors — movement dysfunction, central sensitisation, deconditioning, and psychological components. We address the physical drivers through NKT and manual therapy while supporting the broader recovery process.

What to Expect at Your First Appointment

A first appointment for back pain at Hopevana is around 30 minutes. Here’s exactly what happens.

1

Detailed History

We start with a thorough discussion of your back pain — when it started, what makes it better or worse, your work and activity demands, previous treatment, and your goals. This shapes the entire assessment.

2

NKT Movement Screen

The NKT assessment involves a series of specific manual muscle tests that identify which muscles are functioning correctly and which are inhibited or over-firing. This is where we map out the actual movement dysfunction driving your pain.

3

Hands-On Treatment

You receive treatment at your first appointment — manual therapy, fascia release, and targeted muscle work based on what the assessment revealed. You don’t go home with just exercises and a follow-up appointment; you feel meaningful change before you leave.

4

Personalised Plan

Before you leave, you receive a clear plan: how many sessions we expect you’ll need, the home exercises to start, what activities to modify, and when we’ll review progress. No mystery, no open-ended treatment.

Back Pain Physiotherapy — Frequently Asked Questions

Tap any question to read the full answer.

No — physiotherapists in Australia are primary contact practitioners, which means you can book directly without a GP referral. However, you’ll need a GP referral if you want Medicare bulk billing under a Chronic Disease Management (CDM) or Enhanced Primary Care (EPC) plan, or if your back pain is being treated under WorkCover, CTP, NDIS, or DVA.

It depends on chronicity and presentation. Acute back pain (less than 6 weeks) often responds within 4 to 6 sessions. Chronic back pain that has been present for months or years usually needs 8 to 12 sessions of consistent treatment to see meaningful, lasting improvement. Your physiotherapist gives you a clear estimate at your first appointment based on the NKT findings.

NKT is a manual muscle testing system that identifies movement dysfunction at the neurological level. Where standard physiotherapy assessment looks at where you have pain and what hurts, NKT looks at which muscles are firing correctly and which are inhibited or compensating. For back pain, this often reveals that the back itself isn’t the source — it’s compensating for weak glutes, restricted hips, or poor core sequencing. Finding that compensation pattern is what makes the treatment results last.

Manual therapy can occasionally be uncomfortable — particularly when releasing muscles that have been chronically tight or working in fascia layers that are restricted. We always work within your tolerance, and the discomfort during treatment is far less than the back pain you came in with. If anything is too intense, you tell us and we adjust.

Yes — all major Australian private health funds cover physiotherapy as part of their extras coverage. We have HICAPS on-site at both clinics, so you only pay the gap (if any) at the time of treatment. Bring your private health card to your appointment.

Not usually. Most back pain doesn’t require imaging, and MRI findings often don’t change treatment. Imaging becomes important if you have specific red flag symptoms — significant nerve symptoms, bladder or bowel changes, history of cancer or unexplained weight loss, or significant trauma. If imaging is genuinely indicated based on your assessment, we’ll let you know and recommend you see your GP.

Yes — workplace back injuries are commonly treated under WorkCover, and motor vehicle accident back injuries (including whiplash with referred back pain) are covered under CTP. Hopevana is a SIRA-approved WorkCover provider (Approval No. 15827). For both schemes, you pay $0 out of pocket and we handle all the paperwork directly with your insurer.

Yes — if back pain is part of a permanent or significant disability covered by your NDIS plan, physiotherapy is a fundable support. We are a registered NDIS provider for Self-Managed and Plan-Managed participants. We also offer NDIS home visit physiotherapy across Western Sydney for participants who can’t easily travel to clinic.
